HOW TO APPLY FOR A PASSPORT_________________________________________(click to expand)

1. Confirm you meet the requirements

Apply in person if you respond "Yes" to at least one of the following statements:

I am applying for my first U.S. passport.
I am applying with my child who is under age 16.
My most recent U.S. passport was issued when I was under age 16.
My most recent U.S. passport was lost, stolen, or damaged.
My most recent U.S. passport was issued more than 15 years ago.

2. Fill out your form and print it--> US Passport Application Form

Fill out your form online and then print it.

Tips to complete your form:

Complete all parts of the form. DO NOT SIGN YOUR FORM until asked to do so by a passport acceptance agent or employee.
Print your form on single-sided paper. We do not accept double-sided forms.
Check the information you entered if you did not get the form you expected to receive on our form filler.
You can apply for a passport book, a passport card, or both documents using the same form.
You may request a larger passport book with more visa pages, at no extra cost, by checking the 'large book' box at the top of the DS-11.

3. Bring evidence of U.S. citizenship

Your evidence must be an original or replacement copy. The document must have the official seal or stamp of the office which issued it. You must submit one of the following documents:

U.S. birth certificate that meets the following requirements:
- Issued by the city, county, or state of birth
- Lists your full name, date of birth, and place of birth
- Lists your parent(s)' full names
- Has the date filed with registrar's office (must be within one year of birth)
- Has the registrar's signature
- Has the seal or stamp from the city, county, or state which issued it
Consular Report of Birth Abroad or Certification of Birth
Certificate of Naturalization
Certificate of Citizenship
Full validity, undamaged U.S. passport
- Full validity means the document is or was valid for 10 years for adults and 5 years for children under 16.

4. Bring a photo ID

Bring a physical, photo ID and a photocopy of it. If your photo ID is from a different state than the state in which you are applying, bring a second photo ID.

You must show at least one of these photo IDs:

Valid or expired, undamaged U.S. passport book or passport card
In-state, fully valid driver's license or enhanced driver's license with photo
Certificate of Naturalization
Certificate of Citizenship
Government employee ID (city, county, state, or federal)
U.S. military or military dependent ID
Current (valid) foreign passport
Matricula Consular (Mexican Consular ID) used by a parent of a U.S. citizen child applicant
U.S. Permanent Resident Card (Green Card) used by a parent of a U.S. citizen child applicant
Trusted Traveler IDs (including valid Global Entry, FAST, SENTRI, and NEXUS cards)
Enhanced Tribal Cards and Native American tribal photo IDs
Note: you may need to show another ID when presenting one of these three documents:
- In-state, fully-valid learner's permit with photo
- In-state, fully-valid non-driver ID with photo
- Temporary driver's license with photo

5. Determine special circumstances

You may need to provide more information and documents if:

You are applying for a child age 0-15
Your child, who is age 16-17, is applying
Your valid passport is lost or stolen (you must also submit Form DS-64)
Your valid passport is damaged (include the damaged passport, and a signed statement explaining the damage)
- Normal "wear and tear" such as folded pages or a small bend do not count as damage. Damage includes stains from a liquid, a significant tear, unofficial markings on the data page, missing visa pages (torn out), or a hole punch.
You owe over $2500 in child support
You are selecting a new gender marker
You are applying for a special issuance passport
You are a sex offender covered under International Megan's Law

6. Provide a current photo of yourself

You must provide one photo with your application. PHOTO SERVICES ARE NOT AVAILABLE AT THE CLINTON COUNTY COURTHOUSE.

Do not attach or staple your photo to the form. The acceptance agent or passport employee will review the photo and staple it to your form.
You can get a photo at:
- Some passport acceptance facilities
- Companies which offer photo services in Clinton County:
  - CVS Lock Haven
  - AAA Insurance and Travel Agency-Dunnstown
  - United State Postal Service (Mill Hall Location)
- Home. Ask a friend or family member to take your photo. Print it on glossy or matte photo quality paper.

(You can apply for either the book or card, or both documents.)

International travel by air, sea or land.			Valid for land and sea entry from Canada, Mexico, Caribbean, and Bermuda.

Costs will depend on: Your age, if you want a passport book, card or both; how fast you need your passport, and where you apply. Emergency, expedited, or urgent processing times will have additional costs.

You will need to pay both an application fee and an acceptance fee. Pay the application fee to the U.S. Department of State, and the acceptance fee to the facility which takes your application. To pay for the application and special services such as expediting your application, submit a check (personal, certified, cashier's, traveler's) or money order payable to "U.S. Department of State" To pay the separate acceptance fee: there is a $35 fee per application payable to the "Clinton County Prothonotary" by cash, check or money order. .

FREQUENTLY ASKED QUESTIONS

How do I get the status of my application?

Visit the Department of State's webpage and enroll in emails using the Online Passport Status System.

Application Status	Description
Not Available	This update may mean your application is: Not yet at a passport agency: It may take up to 2 weeks from the day you apply until your application status is "In Process." If you applied in person or renewed by mail, your application is delivered to a mail facility, your payment is processed at an intake facility, and your application is entered into our system and sent to us. If you renewed online, your payment is processed and your application is entered into our system. In our system, but we are experiencing technical issues: If you see "Not Available" after 2 weeks, we may be experiencing a temporary, technical issue. Wait several hours and try to check your status again. In our system, but you need to re-enter your name: If you have a hyphen or apostrophe in your name, enter it with and without the hyphen or apostrophe. Enter all suffixes in your name.
In Process	Your application is being reviewed at one of our agencies or centers. The length of time your application remains In Process depends on the service you selected and our current processing times.
Approved	We have finished reviewing your application and will begin printing your completed passport.
Passport Mailed	We are sending your passport to the address you provided on your application. Check our Online Passport Status System for the tracking number if you applied for a passport book. Save or write down your tracking number now. Future status updates will not include your tracking number.
Supporting Documents Mailed	We sent your supporting documents via First Class Mail to the mailing address you provided on your application. These documents include a passport, birth certificate, or naturalization certificate.

I am traveling very soon. How do I get a passport quickly?

We offer four types of service:
- Life-or-Death Emergency (travel in less than 14 days) (appointment required)
  - You may qualify for a life-or-death emergency appointment if you need to travel to a foreign country in the next 14 days because your immediate family member outside of the United States:
    - Has died; or
    - Is dying (hospice care); or
    - Has a life threatening illness or injury.

- Urgent Travel (travel in less than 2-3 weeks) (appointment required)
  - Choose this service if you are traveling in less than 2-3 weeks.
    
    If you have not applied, make an appointment online. Your appointment at a passport agency or center must be within 14 calendar days of your international travel date.
    
    If you have already applied, call the U.S. Dept. of State at 1-877-487-2778.
- Expedited (travel in less than 8 weeks)
  - Choose this service if you are traveling in less than 8 weeks from the date you submit your application.
    
    Expedited service takes 2-3 weeks and does not include mailing times. Mailing times may add 2 weeks on to the total time to get your passport.
    
    You can expedite at an acceptance facility or expedite a renewal by mail.
- Routine (travel in 8 weeks or more)
  - Choose this service if you are traveling over 8 weeks from the date you submit your application.
    
    Routine service takes 6-8 weeks and does not include mailing times. Mailing times may add 2 weeks on to the total time to get your passport.
    
    You can apply for routine service at an acceptance facility or renew by mail.

Consider the total time it will take to get a passport when you are booking travel. Processing times + mailing times (up to 2 weeks) = total time to get a passport

May I use a passport card to fly?

Can I apply for the passport card and passport book using the same application?

Yes. You can apply for both a passport book and passport card at the same time using the same application. When completing your form, let us know if you would like to apply for one or both documents.

If you're renewing both your passport book and passport card, you must provide both documents. If you can submit a passport book, but you lost your passport card, you cannot renew the passport card using Form DS-82.
