You should first contact the funeral home that provided services for the decedent.  If they will not provide the certificate to you, you may submit an application through your State Representative's office or by visiting the Pennsylvania Department of Health's website at https://www.health.pa.gov/topics/certificates/Pages/Death-Certificates.aspx
